PROCTOR INFORMATION IS OPTIONAL AS WE NOW CAN TEST ONLINE

Just leave blank for online testing! You must find or name a "proctor"
(an independent person to administer your test).
We suggest your town librarian or a person at your town hall (human services).
Simply call and ask if there is someone there that can "administer" a written test
for you - explain what you are doing - or provide us with the phone number(s)
and contact names of your library or town hall and we will arrange it.
Caution - We only send out one test - make sure contact information is correct.

Once you have completed the above applications, complete and submitt payment.
You will be notified within two weeks with confirmation. We will review and process
your application(s) and notify you of the progress. Your membership kit will then be
sent out. If you applied for national certification testing, the process is somewhat
longer because we have to contact your proctor and arrange for the exam to be sent.
We will keep you notified and you will have ample time to plan.
